GS108PEv2 "doesn't exist on network" after firmware upgrade

Finally hooked up an Axis 802.3af network camera to my GS1008PEv2 to take advantage of the POE feature, but camera got no power. First step was to upgrade firmware to 1.00.06 using Prosafe Plus Configuration Utility.

After firmware upload to the GS108PEv2 switch was complete, the switch automatically rebooted as expected, but the switch never came back onto the network.

Restarts of the switch, resets to factory defaults (both by holding in button for >30 second and at powerup) restarts of DM111PSPv2 router manager and restarts of PC's make no difference. Documentation is lacking on this switch, but the 100M port light is blinking rapidly.

I'm inclined to think this is a hardware failure. Submitted an online case to Netgear, will I ever hear back? Case number is: 18984523.
